Palliative care at All Seasons Home Health and Palliative Care supports clients with serious or complex chronic conditions by focusing on comfort, symptom management, and quality of life. Care plans are tailored to individual needs and delivered alongside other home health services.

Services include:

• Pain and symptom management
• Assistance with lifestyle needs and chronic condition support
• Coordination of care with skilled nurses and therapists
• Education and guidance for family caregivers
• Emotional and psychosocial support

You will need to have seen your PCP, surgeon, or specialist 90 days prior to or within 30 days after the start of care before receiving home health care. A Home Health Face-to-Face Encounter document will have to be signed by your health care provider requesting care for you to be in a home health setting. Home Health Care requires a homebound physical status as a necessity to qualify for services.

Medicare pays 100% of the covered services and supplies. We also accept Private Pay, Medicaid, and Insurance Pay.
