The Guiding an Improved Dementia Experience (GUIDE) Model is a voluntary, nationwide model testing the impact of providing comprehensive services and supports for people with dementia and their caregivers. The model began on July 1, 2024, and will run for 8 years.
The program builds a supportive bridge between medical providers and families, introducing four critical components:
- Dedicated Care Navigator: Each family is paired with a specific navigator who creates a personalized care plan, coordinates appointments, manages medication transitions, and locates local resources.
- 24/7 Support Line: Caregivers gain continuous access to a helpline for urgent care questions or emotional support at any time of night or day.
- Up to $2,500 for Respite Care: Eligible caregivers receive up to $2,500 annually to pay for in-home care, overnight respite, or adult day care services so they can take a temporary break.
- Caregiver Training and Education: Clinicians provide training on dementia progression, behavioral management, and logistical tools to make caregiving safer and less stressful.
To participate in the GUIDE program, a patient must meet all of the following rules:
- Diagnosis: Must have a formal physician's diagnosis of Alzheimer’s disease or any stage of related dementia.
- Insurance: Must have Traditional Medicare (Fee-for-Service Parts A and B) as their primary insurance coverage.
- Living Situation: Must live at home or in an assisted living community.
- Exclusions: Long-term nursing home residents, patients enrolled in the Medicare hospice benefit, and individuals enrolled in a Medicare Advantage or PACE plan are not eligible.
